In Line with FA Rules and our child Protection Policy, We require all Managers / coaches / Assistants to hold a valid FA CRB Enhanced Disclosure .
*The FA requires everyone who works with or volunteers in Children's football and regularly cares for, trains, supervises or is in sole charge of children to apply for a FA CRB Enhanced Disclosure*
In order to apply for or renew your CRB Enhanced disclosure, Please contact your Club Welfare Officer. You will ned to fill out an application form and provide the following proofs of identity:
Any 1 item from group 1 and 2 items from group 2
If You cannot Supply 1 item from group 1 then you will require 5 items from Group 2.
Group 1
•Passport any nationality
•UK issued Driving Licence England/Wales/Scotland/Northern Ireland/Isle of Man;either photo card or paper. A photo card is only valid if the individual presents it with thecounterpart licence
•UK Birth Certificate issued within 12 months of date of birth – full or short formacceptable including those issued by UK authorities overseas, such as Embassies,High Commissions and HM Forces.
•EU National Identity Card EU countries only
•UK Firearms Licence
•HM Forces ID Card (UK)
•Adoption Certificate (UK)
Group 2 Issued within last 3 months*
•Mail Order Catalogue Statement*
•Bank/Building Society Statement*
•Addressed Payslip*•Credit Card Statement*
•Utility Bill* electricity, gas, water, telephone – inc. mobile phone contract/bill
•Benefit Statement* e.g. Child Allowance, Pension
•Letter from a Head Teacher*
•Store Card Statement*

•Financial Statement** e.g. pension, endowment, ISA,Mortgage
•P45/P60 Statement (UK)**
•Court Claim Form(UK)** documentation issued by Court Services

•A document from Central/Local Government/GovernmentAgency/Local Authority giving entitlement (UK)* e.g. Department for Work andPensions, the Employment Service, Customs & Revenue, Job Centre, Job Centre Plusand Social Security.
